How SecurePatientForms Works

SecurePatientForms serves as a secure first point of contact between patients and medical providers.

Forms are hosted in a standalone, HIPAA-appropriate environment and can be linked to directly from a provider’s website or embedded where appropriate. Submissions are transmitted securely and delivered directly to a designated HIPAA-compliant email address or intake workflow already in use.

This approach allows providers to receive legitimate patient inquiries without requiring staff to manage additional logins, portals, or third-party dashboards.

Security, Scope & Limitations

SecurePatientForms is designed around data minimization and controlled transmission. Forms collect only limited information necessary for initial contact and are hosted separately from public-facing websites to reduce exposure to tracking technologies and third-party scripts.

Submissions are transmitted securely between HIPAA-appropriate environments and are not intended to support clinical messaging, diagnosis, treatment discussions, or the exchange of detailed medical records.

SecurePatientForms does not replace an electronic health record system or patient portal. Patients are instructed not to submit urgent medical concerns, emergency information, or extensive medical history through these forms.
