Role of SecurePatientForms
SecurePatientForms provides the secure form technology used by individual medical providers.
SecurePatientForms does not provide medical services and does not access, review, or store submitted patient information beyond what is required to securely transmit the submission to the provider’s designated destination.
Medical providers are responsible for determining how submitted information is received, reviewed, stored, and incorporated into their existing intake or communication workflows.
SecurePatientForms stores only structured administrative data in plaintext; narrative medical information is transmitted securely and not retained.
Data Transmission & Hosting
Forms are hosted in a secure, standalone environment separate from public-facing medical websites. This separation helps reduce exposure to analytics scripts, marketing pixels, and other third-party tracking technologies commonly present on public websites.
Information submitted through SecurePatientForms is transmitted using encryption and handled in accordance with applicable HIPAA privacy and security requirements.
Use of Information
SecurePatientForms does not use submitted information for marketing, analytics, profiling, or advertising purposes.
Submitted information is delivered only to the medical provider or destination configured by the provider, such as a HIPAA-compliant email address or secure intake workflow already in use.
